Along with daily home monitoring services from HomMed, registered nurses, licensed practical nurses, certified nursing assistants, medical social workers, and physical, speech and occupational therapists care for patients in the comfort of their own homes on a "visit" basis.
Baptist Home Care is pleased to introduce the Pathways program designed to help patients cope with the life changes associated with chronic illness. The program helps patients develop their own pathway by giving them the tools and assistance to face each challenge.
