Baptist Trinity Home Care & Hospice understands that people face significant health care challenges when diagnosed with a chronic condition. We are pleased to introduce a program designed to help patients cope with the life changes associated with chronic illness.
Our Pathways program helps patients develop their own pathway by giving them the tools and assistance to face each challenge. Our program supports patients by helping them anticipate and plan for the physical, emotional and spiritual changes they may face as their condition progresses.
The goals of the program include:
Supporting patients with chronic conditions by affirming their life goals and helping them maintain or enhance their quality of life
Promoting a holistic, interdisciplinary care process while providing home care to the chronically ill
Using palliative care principles to provide home care to patients with chronic conditions
Providing emotional support and education for patients facing end-of-life issues
The clinical practice model includes the following key components:
Life assessment with input from patients on their life goals and wishes
Interdisciplinary care model
Proactive symptom management
Self-management skills and coping strategies
