What is the HomMed Health Monitoring System?
The easy-to-use HomMed Health Monitoring System performs daily monitoring of home care patients, in addition to patients being monitored through scheduled nurse visits. If any unusual symptoms appear in the daily monitoring reports, a nurse contacts the patient to determine if additional visits are needed to help prevent any serious patient outcomes.

How does it work?
The HomMed system has two parts: a home unit, located in the patient's home or clinical setting, and the central station. At a set time each morning, the monitor automatically turns on and goes through a series of verbal instructions for the patient to follow. The soothing voice from the monitor guides the patient through a three-minute process to collect vital patient information. Immediately, the data are sent by phone modem or wireless transmission to the HomMed central station for evaluation at Baptist.

What data does the HomMed system monitor?
Daily measurements include:
  • Heart rate
  • Blood pressure
  • Mean arterial pressure
  • Weight
  • Oxygen saturation
  • Temperature

What happens to the patient data?
At any given moment, each central station can collect and display information for up to 500 patients. Through easy viewing, a nurse can quickly determine which patients show abnormal symptoms. Tabular and graphical trends can be viewed, printed and faxed to the physician's office for review.

What features does the HomMed system offer patients?
  • A user-friendly, gentle voice and easy-to-read screen that guides the patient through a three-minute vital measurement process; multilingual capabilities include English, French and Spanish
  • A digital scale for quick detection of fluid retention that measures to half-pound accuracy and weighs people up to 500 pounds
  • An easy-to-position blood pressure cuff that measures systolic, diastolic and mean blood pressure
  • An alert that warns a caregiver at the central station if vitals are not sent at the assigned time
  • Guaranteed transmission of information through dual communication modes
  • Easy-to-use format with minimum button presses
  • Digital thermometer that reads body temperature in seconds
  • Compact, lightweight monitor
  • Noninvasive finger monitor that measures heart rate and oxygen saturation

What are some benefits to patients who use the HomMed system?
  • Unobtrusive, easy-to-use home unit
  • Security of daily monitoring
  • Promoting patient participation in plan of care, often improving outcomes
  • Increased patient independence

How does Baptist Trinity benefit from HomMed monitoring?
Telemonitoring lets health care professionals work more efficiently because the system allows one nurse to observe the clinical health of numerous patients at a time and effectively direct medical attention to those who need it. The data are transmitted via digital wireless technology over a virtual private network for review by Baptist Trinity Home Care clinicians.

Who should be considered for HomMed monitoring?
  • Patients with conditions that require constant monitoring or trending of vital signs for treatment
  • Patients with chronic conditions who had a recent hospitalization or ER visit
  • Patients with chronic conditions who are able to stay at home with monitor assistance
  • Patients with chronic conditions who have trouble complying with diet, medication or self-monitoring

Who should not use HomMed monitoring?
  • Patients with behavioral problems that would hinder compliance
  • Patients who are unable to physically or cognitively follow instructions and have no available caregiver
